PTxSA offers mobile concierge physical therapy services for athletes. Our services include movement assessments, manual therapy, rehabilitation, and performance training. All treatment is one-on-one and by appointment only.

2. Eligibility for Services

• You must be at least 18 years old or have parental/guardian consent to receive services.

• Our services are intended for athletes and individuals seeking specialized orthopedic and sports rehabilitation.

• We do not accept insurance; all services are cash-pay or self-pay.

3. Appointment Policies

• Scheduling: Appointments are made via our website, phone, or direct communication.

• Cancellations & Rescheduling:

  • Cancellations within 24 hours of the appointment may result in a cancellation fee (up to the full session cost).

  • Rescheduling is subject to availability.

• No-Show Policy: If you do not show up for a scheduled session without notice, you may be charged the full session fee.

4. Payment & Fees

• Payment is due at the time of service unless otherwise agreed.

• Accepted payment methods include credit/debit cards, digital payment services (e.g., Venmo, Zelle), or direct bank transfers.

• No refunds are issued for completed services.

5. Medical Disclaimer

• PTxSA does not provide emergency medical care. If you experience a medical emergency, call 911 or visit the nearest hospital.

• Physical therapy services are provided by licensed professionals, but results may vary. We do not guarantee specific recovery timelines or performance outcomes.

• You acknowledge that participation in physical therapy involves inherent risks and agree to assume full responsibility for your health and safety.

6. Limitation of Liability

• PTxSA is not responsible for injuries, complications, or adverse effects resulting from treatment.

• We are not liable for loss, damages, or costs related to your use of our services, except in cases of gross negligence or malpractice.

7. Intellectual Property

• All content on our website (text, images, videos, logos, etc.) is the property of PTxSA and may not be copied, reproduced, or distributed without written permission.

8. Privacy Policy

• We collect and store personal information only for scheduling, communication, and service delivery purposes.
